Program Description:
The Adult Day Training Program is for individuals 16+ years that need 1:1, group, and/or respite support services. Services provided are based on evidence-based practices and target person-centered goal development. Additionally, these services offer a weekly customizable schedule. Individuals in the program will engage in daily community activities and learn about and develop social and vocational skills that will allow integration into the community. Funding Sources:

Ideal Candidates:
We have two groups in our Adult Day Training Program. One group targets individuals who may require extra supports for participation in group settings. An individual that wants to work on activities of daily living, but desires an environment that is centered more around social interaction and engagement. If the individual requires personal care supports with eating, mobility, and/or bathrooming, this would be the best fit.
The second group targets individuals that can be successful in a group setting without 1:1 supports and are comfortable to participate in real-life adult conversations and/or scenarios. Individuals that may live with roommates, have a job, or desire to do volunteer work in the future would be the best fit. Individuals who enjoy class-like activities that are centered more around social appropriateness and that require limited, if any, personal care supports (i.e. hygiene, eating, mobility, etc.) would also fit well in this program.

Community Living Supports (CLS)
Due to our new model, we cannot bill CLS on Puzzle Pieces Campus. For those individuals attending Day Training desiring to leave campus with 1:1 services to access the community, we ask CLS and Day Training units be matched. Puzzle Pieces will not exceed the allotted amount of units in combination. CLS will be scheduled for individuals to use the city bus system and/or staff vehicles to access the community to work on individualized goals. Puzzle Pieces Campus does offer CLS to individuals who do not want to attend campus activities and desire for services to be in the community and/or in-home.

*Puzzle Pieces is a vendor of Vocational Rehabilitation for Adult ABA Service, Pre-ETS, and Employment Opportunities. Before Supported Employment can be approved on a plan of care, the individual must first exhaust and use Vocational Rehabilitation funding.

For those who do not have Medicaid Waiver or cannot give Puzzle Pieces the medicaid personal care units the client receives due to PDS support.
